Before you come to Nacogdoches Surgery Center your surgeon will review with you the details of having your surgery performed at the Center and you will be given detailed instructions. The following forms may be completed prior to your scheduled procedure.

• Patient’s Bill of Rights and Responsibilities Form (pdf) - this form will need to be completed and signed in person, at the center
